This is a lovely hand painted wood perpetual calendar with storage compartment for extra tiles by Forever Yours Calendar Creations. I assume this was a kit sold blank for you to decorate as you wanted. Date tiles appear they were written with a marker, the other tiles & frame were done with paint.
